What's Wrong With This Map?

Technically, nothing. It just isn't what you expect.

This map shows the location of one city per state.
It looks like each capital city has been moved to another state.
But there really is a Jackson, Wyoming! And so on.

Which two states are missing? (Easy.)
Which two city names are missing? (Not so easy.)
Which city name is not quite the capital of another state?
